For those over 7 years -
- The Christian Certificate-Student Tester
- Newspaper advertisement - Bank Book
- National / Posture Identification Card - Interim Certificate
- Report from the government or other institution
- Certified copy of voting register
You must submit at least three of the above documents

NB-:
- If the name under the age of 21 is to change the name, the statement should be forwarded by the parent / guardian of their respective children.

- Mother's / father's National Identity Card
- Affidavit that there is no objection to father and mother's protest.
