Timber cutting License

• A copy of the deed certified by the Grama Niladari and verified by the original
• Where the noticeable owner of the land is certified by the Grama Niladari when the applicant is not aware of the clear title of the land
• Affidavit taken to prove that there is no objection to the issuance of a permit to rent to the applicant when there are several heirs
• If a new construction is expected to throw down trees, an appropriate construction plan
• Letters of the pradeshiya Sabhas / Urban Councils if they are being scattered due to dangers
• If court verdicts regarding the land are relevant, court orders regarding them and dump
• Notice of correspondence in the case of power line disturbance or protest
• If the tree is located on the fence, the approximate (bare) land is owned by the owner. The owner is not opposed to cut the tree

Excavation of Sand Gravel

• If the applicant is not the landlord, an affidavit or tax declaration obtained from the landowner should be submitted

Transport of Sand ,Gravel & Soil

• Formally filled application.
• A copy of the mining license of the miner.
• Copy of the Deed of the excavation land and the land obtained

Civil Registration

• Issuing Birth, Death, Marriage Certificates
• Past births (births after three months) Registration
• Registration of past deaths (three months after death)
• Expiration of the name of the birth certificate (below 21 years, over 21 years) / Change (below 21 years, over 21 years)

Issuing of National Identity Cards

• Application for a new Identity Card (New Identity Card - DPR 1, Obtaining a duplicate - DPR 7, Revision of Identity Card - DPR 8)
• Original Birth Certificate or Probable Age Certificate and a photocopy for the applicants who are less than 50 years old
• Age above 50, If there is no birth certificate or probable age certificate, results searching sheet and optional documents should be submitted (certificate of leaving school, certificate of bridging, birth certificate of the birth of the applicant, birth certificate note, estate leave certificate, certified copies of the passport. , Registration card issued by the estate superintendent, birth certificate)

If you are applying for an NIC for the first time, the certified photocopy of the Grama Niladhari's certificate in the National Identity Card of the applicant's parent / guardian.

• Marriage certificate to get husband's name

• Certificate of Service obtained within 3 months if required

For missing identity cards, the original copy of the complaint made by the police

Incorporation/alternation of birth certificate

For those over 7 years -

NB-:

• If the name under the age of 21 is to change the name, the statement should be forwarded by the parent / guardian of their respective children.
• It is recommended that the new name be used for at least one year.

Children below the age of 7 years

• Original birth certificate to be amended
• Parent's Open Certificate
• Mother's / father's National Identity Card
• Affidavit that there is no objection to father and mother's protest.

This declaration should be handed over by the child's parent / guardian

Correction of a mistake that occured in the mother's area of a birth certificate

• Original birth certificate to be corrected
• Original mother's birth certificate
• Copy of mother's national identity card
• Original parents' public certificate

To include/rectify the father's area & civil status in a birth certificate

• The original copy of the birth certificate which can be corrected
• Original father's birth certificate
• Original copy of the parents' certificate

Registration of past births

• Birth Description (Original copy)
• Photo vaccine card copy
• If the parent is married, the marriage certificate of the marriage certificate
• Original Birth Certificates of Parents
• Photocopies of Mother and Father National Identity Cards
• If the birth is less than 1 year, stamps of Rs. 50
• 100 stamps if more than 1 year

Please bring the above documents to the District Registrar of the Department with Parent and child.

Registration of past deaths

• Original death certificate of the hospital if died in a hospital
• Certificate of Grama Niladhari
• Condemnation / death ads
• Cash receipt
• Letter from the Chief Incumbent of the temple
• If the death is less than 1 year, if the stamp is more than Rs. 50 / sterling over 1 stamp
• Birth certificate of the person / marriage certificate and a copy of the National Identity Card
